Florida Concealed Weapons / Firearms License applicants will need the following application requirements :

1. A check or money order for $97.00 (application fee plus background check fee) payable to the Florida Department of Agriculture and Consumer Services (you can abbreviate it to FDACS)
2. A completed fingerprint card (fingerprint card and fingerprinting offered onsite at this course)
3. Training certificate (given upon successful completion of this course)
4. Proof of U.S. citizenship only if born outside the U.S. (copy of U.S. Passport, Certificate of Citizenship, etc…)
5. Only If you are a green card holder (U.S. Permanent Resident), you must  furnish a copy of your resident alien card.
6. A passport-sized photo with your name and birth date written on the back of the photo (photo services offered onsite at this course)
7. A completed application (application provided at this course)
